IMPORTANT! For registration, you only need to complete one registration form per family, but you MUST complete a health form for each swimmer you are registering.
TO REGISTER:
Complete the registration form and the medical form(s). You can complete and print these out from the links at the bottom of this page.
Look at the following registration reminders and make sure you have done everything required.
Some important registration reminders:
Please be sure to complete the front and BACK of the registration form.
Please make sure that you put down your email address, or write NONE if you have none. You will not be spammed! We use email as a primary communication method!
Please indicate which practice session your swimmers will attend, either day or night. If you are not sure, please select the one you feel you are most likely to attend. You can change once the season starts if things aren't working out.
Please have a parent sign the drug policy on the back. All swimmers who are 13 and over must also sign the WST Drug Policy.
Make sure the parent signs the liability policy on the back.
If your family will not be able to provide the minimum required volunteer time, please sign the volunteer waiver and add the $95 fee to the total registration fees. Anyone who has not met their commitment in the past, will need to pay this fee up front before registering. WST could not function without volunteers. It is OUR team. Please make checks payable to WST or Wellington Swim Team.
Please make sure there is a medical form for EVERY swimmer you are registering.

FEES:
$95 for the first swimmer, $175 for two swimmers, $240 for three, and $40 each for the fourth and thereafter.
We will be charging a $15 late fee for any registration submitted after June 4th, 2008.
If you did not make your volunteer hour commitment for 2007, you will need to pay the $80 volunteer opt-out fee for 2007 before your 2008 registration will be accepted. Speak with a member of the Board if you have any questions about this policy.
